LaSPACE Student Participant Information Form:

  • Email LaSPACE@lsu.edu if you are a student participant on an active LaSPACE-funded project and need the link to the new form.
  • The link to our LaSPACE Student Participant Information Form will be included on the Student Participant List page in all LaSPACE program guidelines starting in 2024. All guidelines are available under the Programs tab. 
  • This form should be completed and submitted by all student participants as soon as they are recruited to a LaSPACE project.
  • You will receive a confirmation email after submitting a completed form. Save this email for your records and forward it to your project’s Principal Investigator to confirm you submitted the form.
  • DO NOT SUBMIT SCREENSHOTS/COPIES TO LaSPACE VIA EMAIL OR IN PROPOSAL DOCUMENTS. This is to protect your Personally Identifiable Information. 

Notes for completing the form:

      • The Project’s Principal Investigator / Project Lead is the faculty/staff member serving as the PI on the proposal. They are the person submitting the proposal to LaSPACE. This is not your academic advisor or some other mentor unrelated to the project. (INTERNS ONLY: this is the faculty advisor who is submitting your recommendation letter)
      • The Project’s Principal Investigator / Project Lead should provide you with their office phone number and email address to answer those questions.
      • The LaSPACE Program should be the program for which you are currently applying/participating in. If working under multiple LaSPACE projects, students must submit a demographic form for each separate project. If you don’t know what the LaSPACE Program name is, contact your Project PI/Lead.
      • The Project Start Date should be the first day of the project’s Period of Performance (PoP). This is not your personal start date on the project. Confirm with your Project PI/Lead if unsure. 
      • Participating Semesters is where you select your expected/completed semesters of participation on the project.
  • If you have any questions, first contact your Project PI/Lead. If they cannot answer your question or if you have other issues, please email us at LaSPACE@lsu.edu. Include your full name, your institution, your Project PI/Lead’s name, and the LaSPACE program name.
